Programs:

  • Community Developmental Disability Program (CDDP)
  • Private pay contracts for individuals, adult foster homes, and group homes

Services:

Functional Behavior Assessment (FBA) – A document that describes the challenging behavior and the underlying causes or functions. Information is gathered by conducting interviews, observations, and a review of behavior-related documents.

Positive Behavior Support Plan (PBSP) – A document created, and ultimately every-changing, after the completion of an FBA. It is evidence-based, emphasizes the development of functional alternative behaviors, uses the least intrusive interventions, and evaluates the effectiveness of behavior supports based on data.

Temporary Emergency Safety Plan (TESP) – A document created prior to the FBA and PBSP that outlines behavior supports necessary to support an individual when health and safety risks are present.

PBSP Maintenance – Reviewing the effectiveness of an individual’s PBSP through a review of data tracking sheets, incident reports, and follow up observations. Maintenance also includes training and updates to the PBSP.
